Cancellations

If you are unable to keep your appointment, please notify us 24 hours in advance to avoid a cancellation charge. There will be a $50.00 fee after two consecutive appointments are missed. For surgery cancellations, we ask you to provide 72 hours advance notification or a fee of 10% of surgery cost will be applied to your account. These charges must be paid prior to scheduling any further appointments.

Financial Policy

We realize that for many these are difficult economic times. The goal of Florida ENT Associates has always been to provide our patients with exceptional and compassionate medical care. We believe that outstanding medical service leads the way in helping to keep health care costs down by providing efficient and cost-effective care. We request that co-payments be paid at the time of service. For insurance policies that have deductibles and/or co-insurance, the charges must be paid in full at the time of service. We are happy to see patients who do not currently have health insurance as we understand the difficulty for many to obtain health insurance. These same policies apply; we expect payment at the time of service. For patients who have tests or any procedures to be scheduled, we encourage you to discuss these costs and payments ahead of time. Delinquency charges of 1.5% monthly on the unpaid balances may accrue (minimum $2.00 monthly charge). Surgical procedures require a surgical deposit equal to your estimated out-of-pocket prior to the procedure. Other tests and procedures require payment on the date of service.

Pre and Post Operative Care

Your physician may require a pre-operative appointment. This appointment includes a physical exam, paperwork and testing. Most post operative exams are included in the surgical fees. One known exception is endoscopic sinus surgery.

Many managed care plans have specific requirements that must be met prior to consultation and treatment. Please check with your insurance company for any special instructions to insure payment for your visit to Florida ENT Associates.

Telephone Calls

The doctor-patient relationship is a privileged one. For this reason we do not encourage giving out information about the patient to others, including family members and spouses.

The doctors do not leave patient rooms to answer phone calls as this would be unfair to the patient being treated. Our staff has been trained to answer most questions. If a doctor must return your call after hours a charge may be billed. Most calls for medical treatment will require an office examination.

Prescription Refills

Physicians will not call in any narcotic medication nor will pain medication be called in after office hours. Please call during regular office hours for refills so your medical records can be reviewed.
